- Interested Candidates may apply ONLINE or OFF- LINE mode in the prescribed format along with attested photo copies of relevant certificates/ testimonials in support of date of birth, qualifications, experience, category (SC/ ST/ PWD), Bank Challan etc. with four recent Passport size Photograph one should affix on the application form and other enclosed with it and two self- addressed envelopes may be forwarded to the “Dy.
